To register, download and print a registration form (Adobe PDF format). The registration form MUST print onto one 8.5 x 11 page. Complete the form (be sure to sign it) and return the form with correct payment to the DSL, PO Box 501, Dublin OH 43017 or drop off at the office, 37 W. Bridge St. Use the gold mailbox outside the office door if after hours.

New players need to include a photocopy of birth certificate with your registration. A signed registration forms with all fees paid is necessary to play.
