Kohl's American
Cup
Recreation Tournament
in Dublin, Ohio
June 1-2, 2013
Hosted by the Dublin Soccer League in conjunction with the City of Dublin
This tournament has been sanctioned (Sanction
Form) by the Ohio South Youth Soccer Association, affiliated with USYSA,
USSF and FIFA.
When: Competition will begin at 8:00 am on Saturday,
June 1, 2013, and end on Sunday, June 2, 2013.
Team registration
Teams must check in Saturday, June 1st at Darree Fields, 6259 Cosgray Rd,
Building #3, Dublin, OH, 43016, at least one hour before their first game.
When checking in, a team coach or representative must provide a copy of the
team's roster with will also serve as a Liability/Medical Release.
Entry Fee:
$200 per team
Where: Darree Fields
Park in Dublin, Ohio
(Do not use recreation field layouts for tournament field locations). The
site is within easy access of many fine hotels and restaurants. A
full-service concession stand will be available. Each game
is officiated by three USSF referees.
What: Each participating team will play a minimum of three
games.
Awards:
All registrants receive a participation certificate. Finalists in each division will receive individual
awards for players and coach.
Who: Applications are invited from
recreational teams. Players must be listed on the roster of the team making
the application and play for one team only.

*If numbers permit, separate U11 and U12 age divisions
will be created. **This is a
combined age group. If numbers permit,
separate U13 and U14/15 age divisions will
be created.
Maximum number of players on any one team's roster:
- U-15 playing eleven-a-side, maximum of 18.
- U-11/12 playing eight-a-side, maximum of 14.
- U-08, 09 and 10 playing six-a-side, maximum of 12.
Players may play for only one team in the tournament. Switching teams
by players is not permitted. All of a team's players must be within the guidelines for the
tournament division age group.
There will be NO EXCEPTIONS to the above player/team requirement.

Print an Entry Form (entry form must
be printed on one page, landscape mode), complete, sign, and mail (with check or money order to Dublin
Soccer League for the appropriate amount; $200 per team) to:

Entry Form must be received by
May 18, 2013.
First Class Postage is 46 cents
- Notification of acceptance will be sent no later
than May 23, 2013.
Inclement weather is always a possibility and such
weather may result in abbreviated or cancelled games. In the event of
tournament cancellation due to inclement weather, there is no guarantee of a
refund.
